UnitedHealth Group is seeking a Pride+ Care Adult Primary Healthcare Care Facilitator to support LGBTQIA+ patients in our Worcester/Leominster community. You will coordinate care, assemble plans, and liaise across primary care teams to ensure accessible, respectful services and timely referrals.

This role emphasizes building trusted relationships, navigating insurance and programs, and data management to report on outcomes and performance.

Qualifications

• High School Diploma or GED required.
• 1+ year of experience in a healthcare setting serving patients.
• Experience working with LGBTQIA+ populations preferred.

Responsibilities

• Serves as a primary point of contact for LGBTQIA+ patients and guides their healthcare journey.
• Coordinates care plans across primary care, specialists, and social work teams.
• Manages outreach, referrals, and appointment scheduling.
• Documents outreach attempts in electronic medical records.
• Performs patient outreach calls, mailings, and follow-ups as needed.
• Supports patients in navigating insurance and medication assistance.
