You will work with a multidisciplinary team, link clients to housing, transportation, and healthcare resources, and provide in-home support; you will advocate for clients in offices, hospitals, and at home.

Qualifikationen

• Associates degree in Social Services or 3–5 years of related experience.
• Proof of Covid vaccines: completed original series and booster.
• Proof of negative or inactive TB; TB test within 90 days accepted.

Aufgaben

• Collaborate with RNs, MDs, MSWs and LCSWs to support clients and address social determinants.
• Link clients with resources such as housing, transportation, and healthcare.
• Provide supportive services in clients' place of residence.
• Advocate for clients by accompanying them to medical offices, specialists, hospitals and homes.
• Help develop client care plans and connect to resources per the Case Manager's plan.
• Assist clients facing homelessness, substance use, mental health issues, and chronic conditions.
• Maintain data tracking for BHN position.
• Perform other duties as assigned.
