Job summary

Optum is seeking a Pride+ Care Facilitator to join Reliant Medical Group’s LGBTQIA+ Adult Primary Care team in Massachusetts. The role focuses on coordinating care, maintaining longitudinal relationships, and assessing clinical and social needs to improve health outcomes.

Onsite position in Worcester or Leominster with up to 25% travel; schedule Monday–Friday 8am–6pm EST, with opportunities for community outreach and program development within Pride+ Care.

Qualifications

• High School Diploma or GED required.
• 1+ year experience in a healthcare setting with direct patient contact.
• Preferred: college degree in social work or population health management.
• 1+ year EMR experience preferred.
• Experience with referrals and population health management helpful.
• Experience working with LGBTQIA+ community in coordinator roles helpful.

Responsibilities

• Serve as primary contact for LGBTQIA+ patients, guiding them through care.
• Advocate for patient needs and rights within healthcare systems.
• Collaborate with case management, social work, and insurers as needed.
• Track adherence to appointments, labs, and referrals.
• Coordinate care across primary care and Pride+ Care services.
• Reach out to patients via calls, mailings, and messages for follow-ups.
• Document outreach attempts in the EMR to maintain accurate records.
• Participate in care improvement activities and team meetings.
