Responsibilities

• Performs initial history and physical and documents in EMR.
• Writes standard orders and orders for pre-existing conditions.
• Evaluates patient progress and adjusts treatment as needed.
• Initiates referrals to ancillary departments and community resources.
• Documents progress notes and treatment plans.
• Educates patients and families on health and care.
• Plans home care instructions when needed.
