• Assumes responsibility for the initial evaluation and treatment of patients in assigned areas.
• Performs initial history and physical and documents in the EMR.
• Writes standard orders in accordance with hospital policy.
• Writes orders related to pre-existing conditions.
• Discusses findings with providers and staff as needed.
• Evaluates patient progress and adjusts treatment as needed.
• Assesses medical, social and educational needs of patients.
• Initiates referrals to ancillary departments and community resources.
• Documents progress notes and treatment plans for each visit.
• Establishes diagnoses in compliance with contract.
• Refer to emergency department or specialists for acute conditions as needed.
• Educate patients and families; provide health teaching and discharge instructions.
• Plan home care and community resources as needed.
• Pursue ongoing education and mandatory in-service training.
