Job summary

  • Minimum of 1 year of recent pediatric experience.
  • Minimum of 1 year of demonstrated nursing leadership experience.

Responsibilities

  • Coordinate nursing care and services for patients and families across telephonic, electronic, in person, and mail encounters.
  • Consult with physicians and other providers to resolve health concerns and ensure continuity of care.
  • Lead the patient care delivery team for a specific patient population and assess, plan, and evaluate care.
