Merakey seeks a PQI Coordinator in Philadelphia to advance incident management and ensure timely state reporting. You will manage incident notifications, coordinate with QCO/HR, and maintain accurate data while meeting deadlines.

Strong writing, verbal communication and time management are essential for success. The role involves tracking incidents from initial report to closure and supporting surveys, inspections, and audits, with occasional on-call weekend duties.

Qualifications

• Bachelor's degree with 2+ years in Human Services, or associate's with 4 years.
• Experience in incident management is preferred.
• Investigator Certification may be required by state.

Responsibilities

• Receive notifications of an incident and follow protocols.
• Notify the QCO and HR with initial incident details.
• Track incidents from reporting to closure.
• Coordinate with investigators to meet deadlines.
• Provide weekly updates to PQI Director.
• Monitor and analyze incident data in a centralized system.
• Complete incident reporting audits and related activities.
• Support programs during surveys/inspections.
