Responsibilities

Collects pertinent assessment data from patient and care team to identify needs.
Develops, reviews and revises an individualized care plan with measurable outcomes.
Evaluates patient response and documents changes in condition and interventions.
Delivers teaching using the nursing process and documents outcomes.
Prepares for discharge/transfer and documents the process.
Delivers patient care based on pathophysiology and nursing principles; prioritizes by acuity.
Communicates with patients, families, physicians and staff; participates in rounds.
