Job summary

Qualifications

• High School Diploma or GED required.
• 1+ year experience in community outreach, health education or social services.
• Texas DSHS CHW credential (or obtained within 6 months).
• Associate degree preferred for enhanced eligibility.

Responsibilities

• Conduct outreach and engagement with postpartum mothers.
• Educate participants on health and social services available.
• Screen for social needs and barriers to health.
• Provide culturally responsive support and care coordination.
• Assist with insurance, housing and food assistance applications.
• Document interactions in the study records and ensure follow-up.
