Responsibilities

• Facilitate functional mental health assessments and determine resources for Jericho program.
• Complete Community Linkage Plans for Jericho clients.
• Provide face-to-face and community-based care coordination (billable where possible).
• Facilitate referrals to mental health resources and community programs.
• Ensure Jericho funds are used for critical unmet needs (housing, meds, etc.).
• Provide transportation when clinically appropriate.
• Participate in Jericho Roundtable meetings and coordinate with Shelby County and partners.
