Job summary

College Medical Center Long Beach is seeking a Discharge Planner to coordinate care along a continuum, facilitate referrals, and ensure effective utilization of resources. You will collaborate with providers, patients, and families to craft durable discharge plans and maintain accurate documentation.

The role requires active participation in treatment team meetings, ongoing research to keep resources up to date, and clerical support for Case Managers and Social Workers.

Qualifications

• B.S. Sociology / Psychology or related field required; minimum 1 year in a behavioral health environment.
• BLS (American Heart Association) for healthcare providers upon hire and CPI certification within 30 days of hire.

Responsibilities

• Coordinate referrals, follow-up appointments, and admissions to other facilities.
• Maintain accurate records and ensure smooth communication with community resources.
• Participate in treatment team meetings and ongoing research to keep provider and resource information up to date.
• Provide clerical support to the Case Managers and Social Workers.
