Advanced Medical Management, Inc. is seeking an Outpatient Care Coordinator in Long Beach, CA.

This full-time clerical role supports day-to-day care management by coordinating non-clinical activities, communicating with members, providers, and facilities, and ensuring timely follow-up after hospital or facility discharge. You will collaborate with Case Managers to improve transitions of care, reduce readmissions, and maintain accurate documentation in line with policies and health plan

Qualifications

• Experience communicating with patients, providers, hospitals, and healthcare teams.
• Proficiency with Microsoft Office and learning electronic health record systems.
• Strong organizational, time-management, and documentation skills.
• Ability to manage multiple priorities in a fast-paced team environment.

Responsibilities

• Coordinate services for Care Management program members in line with policies and workflows.
• Monitor discharges from inpatient facilities to support timely post-discharge follow-up.
• Assist with intake and enrollment, outreach, and routing of clinical needs to Case Managers.
• Perform post-discharge outreach and document outreach attempts and outcomes in the system.
• Collect information on medication reconciliation, follow-up appointments, and equipment needs; escalate as needed.
• Collaborate with Case Managers to support transitions of care and reduce avoidable readmissions.
• Serve as a non-clinical contact for members, families, providers, and facilities; route clinical questions appropriately.
• Obtain medical records, discharge summaries, and documentation to support care coordination.
• Upload, index, and maintain records per privacy requirements.
