Job summary

AMM is seeking a Case Management Manager in Long Beach, CA to lead the CM team and ensure high-quality patient care. The role reports to Senior VP Clinical Operations and collaborates with the Client Medical Director on clinical guidance and program development.

The manager will oversee both IP and OP case management, drive discharge planning, disease management, and care coordination initiatives while mentoring staff and ensuring HIPAA compliance.

Qualifications

• Bachelor’s degree required; Master’s Degree in Nursing or Health–Related Major preferred.
• Minimum of 5 years of UM and CM experience in an IPA/PMG or HMO setting.
• Experience in management (hiring, counseling, terminations).
• Maintains confidentiality and adheres to HIPAA policies.
• Managerial skills necessary to independently manage on-site as well as remote team.
• Competent leadership and effective communication style with proven ability to influence behavior.

Responsibilities

• Review Inpatient cases and provide guidance to IP case managers for reviews, transfers, discharge planning, and transitions of care.
• Review outpatient cases and provide guidance to OP case managers for reviews, program overview and disease management.
• Inspire the team to find ways to keep patients out of the hospital and improve care quality.
• Identify, develop and oversee educational needs of the team and motivate goal achievement.
• Collaborate to enhance case management systems and participate in platform development projects.
• Recruit and retain CM team members; conduct annual performance appraisals; provide coaching.
• Participate in budget planning and analyze staffing ratios and productivity.
