Qualifications

• Clinical knowledge of chronic disease management and post-acute care processes.
• Proficiency in motivational interviewing and patient education.
• Strong verbal and written communication with interdisciplinary teams.

Responsibilities

• Post-discharge outreach within 24-72 hours to assess needs and gaps in care.
• Coordinate follow-up appointments and services with care teams and community resources.
• Develop individualized care plans based on program categories and risk factors.
• Document interactions and interventions accurately in EMR/care management systems.
