Job summary

Sanitas is seeking a Case Management Coordinator to manage transitions of care post-discharge and for patients with chronic conditions. You will coordinate follow-up care, educate patients and caregivers, and develop individualized care plans to prevent readmissions and improve health outcomes.

The role requires strong clinical knowledge, experience in care coordination, and proficiency with EMR systems. This on-site position emphasizes collaboration with providers and community resources.

Qualifications

• Strong understanding of chronic disease management and post-acute care processes.
• Proficiency in motivational interviewing to educate and empower members and caregivers.

Responsibilities

• Conduct timely post-discharge outreach (within 24-72 hours) to assess needs and gaps in care.
• Coordinate follow-up appointments and post-acute services across providers.
