Job summary

Sanitas seeks a Case Management Coordinator to manage transitions of care after discharge and for patients with chronic conditions. You will coordinate follow-up services, create care plans, and educate patients and families to prevent readmissions and improve health outcomes.

Responsibilities include timely outreach, collaborating with providers and home health agencies, and documenting all actions in the care management system in compliance with guidelines.

Qualifications

• High school diploma or equivalent.
• Graduation from a nationally accredited MA program.
• Active MA national certification.
• CPR/BLS for Healthcare Providers required.

Responsibilities

• Post-discharge outreach within 24-72 hours to assess needs and gaps in care.
• Coordinate follow-up appointments with providers and home health services.
• Develop individualized care plans and educate patients and caregivers.
• Document interactions and interventions in care management platforms.
• Escalate clinical issues when needed and monitor for readmission risk.
