Job summary

Qualifications

Responsibilities

• Post-discharge outreach and gap assessment within 24-72 hours.
• Coordinate follow-up appointments and post-acute services with providers and home health.
• Develop individualized care plans and monitor chronic conditions.
• Educate members and caregivers using motivational interviewing techniques.
• Document interactions in EMR/care management platforms to ensure compliance.
