Job summary

Sanitas is seeking a Case Management Coordinator in the Phoenix region to manage post-discharge transitions and support patients with chronic conditions. You will coordinate follow-up care, develop individualized care plans, and educate patients and caregivers to prevent readmissions.

You will collaborate with primary care providers, specialists, and community resources while documenting care plans in the EMR and ensuring adherence to clinical guidelines.

Responsibilities

• Educate patients using motivational interviewing to reduce unnecessary visits.
• Document all assessments and interventions in EMR/care management platforms.
