Job summary

Sanitas seeks a Case Management Coordinator to manage transitions of care, coordinate follow-up services, and educate patients after discharge to prevent readmissions. Responsibilities include developing care plans, identifying risks, and collaborating with primary care, specialists, and home health teams.

Required: MA program completion, active MA certification, CPR/BLS, and strong communication skills. Bilingual English/Spanish preferred; hybrid work in Phoenix area is offered.

Qualifications

• High school diploma or equivalent and completion of an accredited MA program.
• Active MA national certification and CPR/BLS for Healthcare Providers.
• Experience in clinical case management or care coordination preferred.

Responsibilities

• Post-discharge outreach within 24-72 hours to assess needs and understanding.
• Coordinate follow-up appointments and post-acute services across providers.
• Develop individualized care plans based on program categories and risk factors.
• Educate members/caregivers using motivational interviewing techniques.
• Document assessments and interventions accurately in EMR/care management systems.
