Sanitas is seeking a Case Management Coordinator in the Phoenix region (Surprise, AZ area) for a hybrid, full‑time role with up to 50% travel. The position focuses on managing transitions of care, coordinating post-discharge follow-up, and education to prevent readmissions and improve outcomes.

The role requires MA program completion and active MA certification, with strong clinical knowledge and communication skills. Travel and bilingual abilities are advantageous for serving diverse patients.

Qualifications

• High school diploma or equivalent; completion of an MA program is required.
• Active MA national certification and CPR/BLS for Healthcare Providers.
• Clinical case management, care coordination, utilization review, or population health management experience.
• Advanced English; bilingual in Spanish/Creole preferred.

Responsibilities

• Perform post-discharge outreach within 24–72 hours to assess needs and gaps in care.
• Coordinate follow-up appointments and post-acute services across care teams.
• Develop individualized care plans and identify early signs of complications for chronic conditions.
• Educate members and caregivers using motivational interviewing to improve self-management.
• Document assessments and interventions accurately in EMR/care management systems.
