Job summary

Lund is seeking a Post Adoption Case Manager to support families formed through adoption. The role emphasizes trauma-informed parent coaching, access to adoption-competent providers, and education on attachment and lifelong needs.

You will advocate for families in schools, facilitate monthly support groups, and coordinate community resources. The position is full-time (40 hours/week), home-based with in-person work in Rutland County, VT, and requires regular travel within the county.

Qualifications

• Bachelor’s Degree in Human Services field.
• 2 years of trauma/adoption-related work or transferrable experience.
• Strong writing, organizational and interpersonal skills.

Responsibilities

• Provide home visits and organize monthly support groups.
• Advocate for families in school and team meetings.
• Connect caregivers with training and community resources.
