Job summary

Lund seeks a Post Adoption Case Manager to support families formed through adoption. This full-time role emphasizes coaching, education on early trauma and attachment, and advocacy in school settings to promote stability and resiliency in Vermont families.

The position requires home-based work with in-person visits in Rutland County and regular travel within the county. A focus on coordinated care and access to resources is key.

Qualifications

• Bachelor’s Degree in Human Services field.
• Preferred 2 years of professional experience in trauma and adoption-related work or an equivalent of transferrable professional and lived experience.
• Strong writing, organizational, interpersonal and team skills.
• Ability to obtain VT Driver’s License and access to reliable transportation.

Responsibilities

• Provide parent coaching and support for families formed through adoption.
• Educate parents on early trauma, attachment, and lifelong needs.
• Advocate for families in school and team meetings.
• Organize and facilitate monthly support groups.
• Conduct home visits and connect caregivers with training and community resources.
• Travel throughout Rutland County to meet with clients.
