Qualifications

• LCSW, LSW, or MSW license required.

Responsibilities

• Advocate for residents and protect their best interests.
• Assess psychosocial status and communicate findings to care teams.
• Maintain clinical documentation for residents referred to social work.
• Coordinate referral services for residents and families.
• Provide information on community resources and services.
• Serve as liaison between residents, families, and community agencies.
• Participate in care planning and case conferences for residents.
