Responsibilities

• Assesses psychosocial status and communicates findings to the nurse.
• Carries out social evaluations and person-centered intervention plans.
• Maintains clinical documentation on all patients referred to social work.
• Coordinates referral services for residents/patients and families.
• Provides information to residents/patients or families regarding community resources.
• Acts as liaison between residents/patients and community agencies.
• Works with the care team to develop the total plan of care and participate in conferences.
• Assists in understanding social and emotional factors related to health problems.
• Establishes behavior monitoring and interventions to provide quality care.
• Addresses resident behaviors to prevent harm.
