Job summary

Olde Main Post Acute is seeking a Director of Social Services to lead the psychosocial care team, ensuring compassionate support and advocacy for residents and families. This role oversees programs, drives compliance with regulations, and coordinates care across disciplines within a nursing facility.

The position emphasizes collaboration with healthcare professionals, discharge planning, and resource coordination, with an emphasis on dignity and emotional well-being for residents and families.

Qualifications

• Bachelor’s degree required; master’s degree strongly preferred.
• Current LSW/CSW licensure required.
• Experience as a social worker in a healthcare setting is required.
• Experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, standards, and guidelines governing nursing care facilities.
• Excellent communication skills with the ability to interact effectively and compassionately with residents, families, staff, and external agencies.
• Ability to balance strong advocacy with empathy, professionalism, and emotional support.

Responsibilities

• Provide leadership and oversight of social services programs to support residents and families.
• Ensure services are delivered in accordance with federal, state, and facility regulations.
• Collaborate with interdisciplinary teams to support resident-centered care planning.
• Provide compassionate counseling, support, and advocacy for residents and families.
• Serve as liaison between residents, families, staff, and external agencies for coordinated care.
• Support staff in addressing resident concerns, behavioral challenges, and psychosocial needs.
