Job summary

Newport Rehabilitation of Cascadia seeks a Social Services Director to advocate for residents and ensure their best interests are observed while assisting with remedies. The role involves coordinating care, discharge planning, and collaboration with the care team in a Newport, OR facility.

The position requires a social work degree and an active SW license, with post-acute or skilled nursing experience preferred, and a valid driver’s license. Comprehensive benefits are offered.

Qualifications

• Bachelor’s or Master’s degree in social work.
• LCSW, LSW, or MSW license required.
• Must have a valid driver’s license.
• Experience in post-acute/ skilled nursing preferred.

Responsibilities

• Assess the psychosocial status of patients and communicate findings to the nurse.
• Carry out social evaluations and plan person-centered interventions.
• Maintain clinical documentation for social work referrals.
• Coordinate referral services for residents and families.
• Provide information on community services to residents and families.
• Serve as liaison between residents and community agencies.
• Collaborate with staff to develop the total care plan and participate in conferences.
• Assist in discharge planning and post-discharge needs.
• Coordinate medication review to minimize dose where appropriate.
