Valley West, part of Life Care Centers of America in Oregon, seeks a Social Services Director to oversee the department and ensure residents' social and emotional needs are met in compliance with laws and Life Care standards.

Requires active state license, Bachelor's degree for facilities with 120+ beds, and at least two years of health care social work experience; duties include advocacy, education, charting, staff supervision, and interdisciplinary teamwork.

Qualifications

• Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.
• Bachelor’s degree if working in a facility with 120 or more beds (see State law).
• Two (2) years’ experience in health care social work preferred

Responsabilités

• Implement a social services program meeting patients' medically related social and emotional needs.
• Act as patient advocate and educate staff on patient rights.
• Chart appropriately and timely; assist patients and families with education and planning.
• Recruit, hire, train, supervise social services staff; collaborate on interdisciplinary team.
