Cottage Grove Post Acute is seeking a dedicated Physical Therapist to join our team. Based in Cottage Grove, you will evaluate patients, design evidence-based treatment plans, and deliver hands-on care to support recovery and mobility.

You will document care per EMR standards, collaborate with a multidisciplinary team, and educate patients on home exercise programs and injury prevention. APTA-PT degree and state license required or eligible.

Formación

• Graduate of an APTA-accredited physical therapy program (Master's or Doctorate).
• Current or eligible State Physical Therapist License.
• Current Basic Life Support (BLS)/CPR certification preferred but not required.
• Strong clinical reasoning and excellent interpersonal communication; team-first mindset.

Responsabilidades

• Conduct comprehensive patient evaluations and develop evidence-based treatment plans.
• Provide hands-on care using manual therapy, therapeutic exercises, and modalities.
• Maintain timely EMR documentation in compliance with regulatory standards.
• Collaborate with physicians, family members, and healthcare team to coordinate care.
• Educate patients and caregivers on home exercise programs and injury prevention.
