Job summary

Gateway Post Acute is seeking a compassionate Physical Therapist to join our rehabilitation team. The role focuses on evaluating mobility and implementing personalized treatment plans to help residents regain independence.

You will collaborate with nursing, therapy, and support staff to deliver meaningfully progressive care, while documenting outcomes and participating in care planning. Competitive hourly pay and comprehensive benefits offered.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current California Physical Therapist license in good standing.
• Current CPR/BLS certification.
• Previous skilled nursing or long-term care experience preferred.
• Strong clinical assessment, treatment planning, and documentation skills.
• Excellent communication, organization, and interpersonal skills.
• Ability to work independently and collaboratively.
• A compassionate approach and commitment to quality resident care.

Responsibilities

• Evaluate residents’ mobility, strength, balance, endurance, and functional abilities.
• Develop and implement individualized treatment plans based on assessments, physician orders, and resident goals.
• Provide therapeutic exercises, gait training, balance activities, and other appropriate interventions.
• Help residents improve transfers, mobility, and safe use of assistive devices.
• Monitor progress and adjust treatment plans to support meaningful outcomes.
• Educate residents, families, and caregivers on exercises, mobility techniques, and fall prevention.
