Job summary

Enhabit Home Health & Hospice seeks qualified health professionals to support patients through post-acute care transitions and improve outcomes. You will apply evidence-based guidelines to develop patient-centered plans and collaborate with health systems and physicians.

The role emphasizes compliance with federal/state regulations and using mobile technology to support care delivery and coordination.

Qualifications

• Must be a graduate of an approved school of nursing, therapy or social work.
• Must be licensed in the state where they currently practice.
• Must have two years' demonstrated field experience.
• Must have demonstrated knowledge of federal, state, and local laws governing Medicare home health and hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Assists patients in navigating post-acute care to improve outcomes and referral source satisfaction.
• Develops transition plans using evidence-based and preventative guidelines to promote quality and efficiency.
• Represents the area branches in strategic relationships with health systems, hospitals, and physicians.

Education

Graduate of an approved school of nursing, therapy or social work

Licensed in the state of practice

Two years of field experience
