Job summary

Enhabit Home Health & Hospice is seeking qualified clinicians to support post-acute care navigation and patient-centered transition planning. You will develop plans that emphasize evidence-based guidelines to improve outcomes and referral satisfaction.

The role involves collaboration with hospitals, inpatient facilities, physicians, and executive-level stakeholders to advance home health objectives and program success.

Qualifications

Must graduate of an approved school of nursing, therapy or social work.
Must be licensed in the state where they currently practice.
Must have two years' demonstrated field experience.
Must understand federal, state, and local laws governing Medicare-certified home health and hospice.
Must have basic technology skills, including operation of a mobile device.

Responsibilities

Assists patients in navigating post-acute care to improve outcomes and referral source satisfaction.
Develops transition plans using evidence-based and preventative guidelines.
Represents the area branches in strategic relationships with health systems, hospitals, physicians, and executive level opportunities.
