PACS Group in the United States seeks a Reserved Bed Post-Acute Care Manager to coordinate referrals, manage patient placement, and maintain program metrics. You will liaise with hospital case management teams and PACS facilities, ensuring compliant, timely transitions for patients.

Key duties include educating partners, reviewing referrals, coordinating transfers, and reporting performance. Remote work is supported with travel as needed to hospital sites.

Competenze

• Must have a high school diploma or GED; bachelor's degree preferred.
• Two years admissions experience preferred.
• LVN or RN license required.

Mansioni

• Educate hospital case managers on the Reserved Bed Program and admission processes.
• Receive Reserved Bed referrals and review records for eligibility.
• Distribute referrals to PACS facilities and track outcomes.
• Assess patient financial resources and insurance coverage for placement.
• Coordinate patient transfers with orders, documentation, meds and equipment.
• Attend hospital case management meetings and maintain partnerships.
• Document referrals and placements in tracking systems; monitor KPIs.
• Identify opportunities to improve processes and grow the program.
• Travel to hospital locations as needed for assessments.
