Job summary

PACS Group seeks a Reserved Bed Post-Acute Care Manager to serve as the liaison between hospital partners and PACS post-acute centers. You will coordinate referrals, placement, and reporting while maintaining program metrics and strengthening hospital relationships.

Responsibilities include educating case managers, reviewing records for eligibility, and managing the referral process with a focus on timely admissions and compliant operations.

Qualifications

• High school diploma or GED required.
• Bachelor’s degree in health care preferred.
• LVN or RN license required.
• Two years admissions experience preferred.
• Proficient reading, writing, and communication skills.

Responsibilities

• Educate hospital case managers on the Reserved Bed Program, referral criteria, and admission processes.
• Receive referrals and review patient records for eligibility.
• Manage referrals, distribute to PACS facilities, and report placement outcomes; escalate unplaced referrals.
• Assess patient financial resources to support placement recommendations.
• Coordinate patient transfers with orders, documentation, medications, and equipment.
• Participate in hospital case management meetings and committee discussions.
• Maintain timely documentation of referrals and admissions in tracking systems.
• Monitor Reserved Bed census and ensure contract compliance.
• Track KPIs and program metrics; report on outcomes.
• Develop relationships with hospital partners and PACS facility leaders.
• Identify process improvements to enhance efficiency and patient outcomes.
• Perform other duties as assigned.
