Job summary

PACS Group, Inc. is seeking a Resident Care Manager in Forest Grove to apply clinical expertise in planning and coordinating resident care. You will collaborate with residents, families, healthcare providers, and payers to deliver extraordinary care while managing costs and resources.

The role emphasizes communication, care planning, and coordination across admissions, discharges, and referrals, with a strong focus on compliant documentation and team collaboration.

Qualifications

• Bachelor’s Degree in Nursing or Social Work; RN license preferred; LVN/LPN acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs.
• CCM certification a plus.
• Continuing education/licensing kept up to date and good standing with relevant authorities.

Responsibilities

• Communicate plans of care with residents and families.
• Attend morning meetings to coordinate team communication.
• Answer questions about care, treatment plans, illness progression, and safety.
• Monitor resident status and adjust plans as needs change.
• Organize daily tasks by prioritizing resident needs and discharges.
• Complete documentation and maintain accurate records.
• Conduct utilization reviews to ensure appropriate, cost-effective care.
• Coordinate referrals with residents, families, care providers, and payers.
• Oversee admissions and discharge activities.
• Promote cooperation among residents, families, staff and partners.
