Job summary

Eastern Healthcare Group is seeking a Hospital Liaison to act as the primary link between hospitals and our skilled nursing facilities. The role focuses on developing referral relationships, facilitating admissions, and ensuring smooth transitions from acute to post-acute care.

You will educate referral sources, conduct hospital visits, and coordinate patient placements while tracking referrals and reporting on performance metrics. A valid driver's license is required for territory management.

Qualifications

  • Bachelor's degree in Healthcare Administration, Marketing, Business, Nursing, Social Work, or related field preferred.
  • Minimum of 2 years of experience in healthcare sales, business development, admissions, case management, or hospital liaison roles.
  • Experience working with skilled nursing, rehabilitation, long-term care, home health, hospice, or hospital systems preferred.
  • Strong understanding of hospital discharge planning and post-acute care services.
  • Excellent communication, relationship-building, and presentation skills.
  • Ability to work independently and manage a territory effectively.
  • Strong organizational and time-management abilities.
  • Valid driver's license and reliable transportation.

  • Participate in networking events, healthcare forums, and community outreach initiatives.
  • Stay informed on Medicare, Medicaid, and managed care guidelines affecting post-acute care services.
  • Meet and exceed census growth and referral development goals.
