Job summary

Kennedy University Hospitals, Inc. is seeking a part-time Case Management support professional to assist with coordinating post-hospital care and service arrangements.

The role involves patient interaction, arranging homecare and DME, and obtaining post-acute authorizations in a fast-paced environment. The ideal candidate has a high school diploma (bachelor's preferred), hospital or patient-facing experience, and strong medical terminology knowledge.

Qualifications

• High School Diploma required; Bachelor's degree preferred.
• Experience in a hospital or patient-facing role preferred.
• Experience obtaining post-acute insurance authorizations via insurance portals.
• Computer literacy and strong knowledge of medical terminology required.

Responsibilities

• Coordinate identified homecare, DME, transportation, infusion services and other post-acute services and authorizations for all identified patients.
• Interact with patients and families at the bedside and over the phone to obtain patient rights documentation.
• Interact with coworkers and staff in alignment with Jefferson values.
• Must be able to multi-task and prioritize the order of work in a fast-paced environment.
