Job summary

Trinity Health Mid-Atlantic is seeking a Part-time Department Assistant to join the Case Management Team at Saint Francis Hospital. The role supports discharge planning, coordinates post-acute referrals, and works with the Inpatient Care Coordination team to ensure smooth patient transitions.

Responsibilities include pulling reports in EPIC, maintaining records, and coordinating transportation for home returns.

Qualifications

• Medical terminology knowledge
• Computer skills and familiarity with healthcare software
• Excellent communication, interpersonal and telephone skills
• Ability to follow confidentiality and HIPAA requirements
• Detail-oriented with strong organizational skills

Responsibilities

• Pull daily reports and census from EPIC on inpatient units
• Maintain accurate and up-to-date documentation in departmental records
• Collaborate with Inpatient Care Coordinators on discharges and rounds
• Communicate with insurance companies for authorizations/denials
• Assist with referrals to skilled nursing facilities and home health agencies
• Coordinate transportation for patients returning home
• Assist with timely patient throughput and post-acute care planning
