Endeavor Health in Evanston, IL is seeking an Advanced Practice Provider (APN/PA) to join its Post-Acute Care team. The role focuses on skilled nursing facility patients in the Evanston region, with an average 40-hour workweek and a Monday–Friday schedule.

• Full Time/Part Time: Full-Time (average of 40 hours/week)
• Hours: Monday – Friday between 8:00AM - 5:00PM
• No evenings or holidays. Potential for weekend hours in the future.

Job Description

• Exciting opportunity to join Endeavor Health Department of Post-Acute Care (Skilled Nursing Facilities )!
• Under the direction of the collaborating physician(s), provides care to patients.
• Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results.
• Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions.
• Maintains records and writes progress notes on status, treatments and procedures.
• Follows patients through their course of treatment in the clinic and hospital setting.
• Collaborates with physicians and the health care team to achieve optimal patient outcomes.
• Prescribes pharmacologic and non-pharmacologic therapies as appropriate.
• Demonstrates evidence-based practice.
• Participates in the development, implementation, and evaluation of standards of practice in the specialty.
• Assesses the learning needs of patients and families across the continuum.
• Employs strategies, interventions and teaching to promote, maintain, and improve health.
• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
• Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities.
• Works autonomously and in interdisciplinary teams as resources and consultants.
• Positively contributes to safety outcomes and promoting high quality patient experience.
• Participates in the development, implementation, and evaluation of service line goals.
• Supports the strategic plan of the organization.
• Adheres to all Endeavor Health policies and procedures.

Success in this position requires strong clinical judgment, the ability to work autonomously, and a collaborative approach to partnering with the interdisciplinary team to develop and execute individualized care plans. As a valued member of our Post-Acute Team, you will play a key role in supporting seamless transitions of care, improving patient outcomes, and ensuring continuity throughout the recovery journey.

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
• Education: Completion of master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience: Prior APP experience preferred.

Preferred Qualifications

Previous experience practicing in a skilled nursing facility or post-acute care setting is highly desirable. Candidates should have an understanding of the unique needs of the geriatric population, including the management of complex medical conditions, rehabilitation, and transitional care. Experience collaborating with interdisciplinary teams within the skilled nursing environment is strongly preferred.
