Endeavor Health in Skokie, IL, seeks an Advanced Practice Provider in Physical Medicine & Rehabilitation for a full-time outpatient role at Skokie Hospital. The position requires 40 hours per week, Monday–Friday with potential weekend availability and travel.

Under physician supervision, you will perform histories, exams, order tests, interpret results, prescribe therapies, and collaborate with the care team to optimize patient outcomes in neurology.

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in IL; Federal DEA and Illinois Controlled Substance License
• Education: Master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience: Prior APN/PA experience preferred

Responsibilities

• Provides care under the direction of collaborating physicians.
• Obtains detailed history and performs physical examinations; orders imaging, labs and diagnostic procedures, and interprets results.
• Instructs patients on prescribed regimens and pre/post operative instructions.
• Maintains records and writes progress notes; follows patients through their treatment course.
• Collaborates with physicians and health care team to achieve optimal outcomes.
• Prescribes pharmacologic and non-pharmacologic therapies as appropriate.
• Demonstrates evidence-based practice and participates in standards development and evaluation.

• Works autonomously and in interdisciplinary teams as resources and consultants.
• Positively contributes to safety outcomes and promoting high quality patient experience.
• Participates in the development, implementation, and evaluation of service line goals.
