Job summary

Endeavor Health is seeking experienced Advanced Practice Providers (APN/PA) for Immediate Care across four northwest Chicago locations, including Schaumburg. Positions are full-time and part-time, with weekend, evening, and holiday coverage as needed.

Providers work under physician supervision, perform comprehensive assessments, order and interpret diagnostics, and guide treatment plans in a fast-paced clinic/hospital setting.

Qualifications

• IL Advanced Practice Licensure (APN) or PA licensure required.
• Master's degree in specialty area.
• BLS/CPR certification required.
• Minimum of two years APP experience in an Urgent Care or Emergency Medicine.

Responsibilities

• Provide care in a team-based environment.
• Obtain detailed history and perform physical examinations; order imaging and labs and interpret results.
• Instruct patients on prescribed regimens and pre/post-operative care instructions.
• Maintain records and write progress notes on status, treatments and procedures.
• Follow patients through their course of treatment in clinic and hospital settings.
• Collaborate with physicians and healthcare team to achieve optimal patient outcomes.

Position Highlights

• Position: Advanced Practice Provider (APN/PA) - Immediate Care
• Location: Four locations throughout the northwest suburbs of Chicago including: 519 S Roselle Road Schaumburg IL, 21481 N Rand Road Kildeer IL, 15 S McHenry Road Buffalo Grove IL, 199 W Rand Road Mt Prospect IL
• Weekends, evenings and holidays may be required
• Under the direction of the collaborating physician(s), provides care to patients.
• Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results.
• Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions.
• Maintains records and writes progress notes on status, treatments and procedures.
• Follows patients through their course of treatment in the clinic and hospital setting.
• Collaborates with physicians and the health care team to achieve optimal patient outcomes.
• Prescribes pharmacologic and non-pharmacologic therapies as appropriate.
• Demonstrates evidence-based practice.
• Participates in the development, implementation, and evaluation of standards of practice in the specialty.
• Assesses the learning needs of patients and families across the continuum.
• Employs strategies, interventions and teaching to promote, maintain, and improve health.
• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
• Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities.
• Works autonomously and in interdisciplinary teams as resources and consultants.
• Positively contributes to safety outcomes and promoting high quality patient experience.
• Participates in the development, implementation, and evaluation of service line goals.
• Supports the strategic plan of the organization.
• Adheres to all Endeavor Health policies and procedures.

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
• Education: Completion of masters level program in specialty area
• Certification: BLS/CPR certification required
• Experience: Minimum of two years APP experience in an Urgent Care or Emergency Medicine.

• S uturing, suture removal, I&D, ear irrigation, pelvic floor exams, POC testing: CBC/BMP, COVID/Flu, Strep, INR, advanced imaging
