Endeavor Health is seeking an Advanced Practice Provider (APN/PA) for Post-Acute Care in the North Region, Evanston area. You will work with a collaborating physician, perform exams, order tests, and manage treatment plans across SNF settings.

Position includes full-time hours, weekday schedule 8:00 AM–5:00 PM, with no evenings or holidays and potential future weekend hours. Active IL APN/PA licensure and DEA are required; requires 3+ years APP experience.

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in IL required
• Federal DEA and Illinois Controlled Substance License required
• Completion of master’s level program in specialty area
• BLS/CPR certification required
• Minimum of three years APP experience; experience with advance directives and goals of care

Responsibilities

• Under the direction of collaborating physicians, provides care to patients
• Obtains detailed history and performs physical examinations; orders imaging, labs and diagnostic procedures and interprets results
• Instructs patients on prescribed regimens and pre/post operative instructions; maintains records and progress notes
• Collaborates with physicians and healthcare team to achieve optimal patient outcomes; prescribes therapies as appropriate
• Promotes evidence-based practice and participates in standards of practice development and evaluation

Positively contributes to safety outcomes and promoting high quality patient experience. Participates in the development, implementation, and evaluation of service line goals.
