Endeavor Health is seeking an Advanced Practice Provider (APN/PA) for post-acute care in skilled nursing facilities. Position involves autonomous patient care within interdisciplinary teams, under physician supervision, with a focus on evidence-based practice and comprehensive health assessments.

Location: Skilled Nursing Facilities at Endeavor North Region
Part-Time/Part Time: Full-Time (average of 40 hours/week)
Hours: Monday – Friday between 8:00AM - 5:00PM No evenings or holidays. Potential for weekend hours in the future.
Required Travel: Deerfield/Lincolnshire/Highland Park territory
Exciting opportunity to join Endeavor Health Department of Post-Acute Care (Skilled Nursing Facilities)! Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities. Works autonomously and in interdisciplinary teams as resources and consultants. Positively contributes to safety outcomes and promoting high-quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures.
