Job summary

Sarasota Memorial Health Care System is seeking a Post-Acute Access Manager to lead the admissions team, manage referrals, and ensure smooth transitions for patients discharged to post-acute services.

The role requires a bachelor’s degree (master’s preferred) with 7+ years in clinical or post-acute settings and at least 2 years in a supervisory role. Certifications such as CHAM and CCM/ACM are encouraged within the first year.

Qualifications

• Bachelor's degree in a related field; master's preferred.
• 7+ years hospital or post-acute setting experience, with 2+ years in lead or supervisory role.
• Certifications within 1 year: CHAM, CCM/ACM and Admissions Liaison (UDS).

Responsibilities

• Oversee post-acute admissions, referrals, and smooth care transitions.
• Ensure compliance with registration, scheduling, and regulatory requirements.
• Provide timely reporting and KPI analysis for the post-acute division.
