Qualifications

• 3 years of experience in population health, quality/process improvement, nursing leadership, or project management.
• Current, valid AZ RN License required.

Responsibilities

• Supports strategic planning for population health plan consistent with organizational priorities.
• Leads and develops content for key committees overseeing population health projects.
• Coordinates with analytics team to develop reporting tools for evaluation.
• Supervises the nurse navigators within outpatient clinics.
