Job summary

Hartford HealthCare Medical Group is seeking a Population Health Specialist to coordinate patient screenings and close gaps in care across Connecticut. The role requires travel between CT sites, training offices on workflows, and auditing training opportunities.

You will support quality initiatives, maintain training documents, and contribute to the MA career ladder program while benefiting from tuition reimbursement, health coverage, PTO, and wellness programs.

Qualifications

• Diploma from an accredited Medical Assistant program.
• Medical Assistant Certification preferred.
• 1–2 years Medical Assistant experience.

Responsibilities

• Ensure patients are up to date with diabetic and routine health screenings.
• Develop and maintain training documents and workflows.
• Train offices across Connecticut to close gaps in care.
• Identify training opportunities through routine audits.
• Travel between Hartford HealthCare offices to conduct trainings.
