Hartford HealthCare Medical Group is seeking a Population Health Specialist to coordinate patient screenings and close gaps in care across Connecticut. The role requires travel between CT sites, training offices on workflows, and auditing training opportunities.

You will support quality initiatives, maintain training documents, and contribute to the MA career ladder program while benefiting from tuition reimbursement, health coverage, PTO, and wellness programs.

Formación

• Diploma from an accredited Medical Assistant program.
• Medical Assistant Certification preferred.
• 1–2 years Medical Assistant experience.

Shift Detail: Monday - Friday, 8-4:30. Travel across CT to different sites and locations required.

Job Summary

The Population Health Specialist works to ensure patients are up to date with their diabetic and routine health screenings. This position supports Hartford HealthCare quality and balanced scorecard initiatives by working to ensure patient gap in care completion, creating and maintaining necessary training documents, and training offices on workflows to close gaps in care. Performs routine audits to identify training and education opportunities for offices where workflows are not being followed. This is a central position in Hartford, but does require travel to offices across Connecticut to perform trainings.

• 1-2 years Medical Assistant experience

Education

• Diploma from an accredited Medical Assistant Program and Medical Assistant Certification preferred.
