Job summary

Sparrow Health System is seeking a Resource Coordinator to support ambulatory care coordination and address social determinants of health for patients. You will connect patients with community resources, collaborate with SMG practices, and help close care gaps through outreach and education.

The role focuses on coordinating referrals, assessing patient needs, and maintaining partnerships with community organizations. A relevant health-related degree and CHW/MICHWA training are preferred.

Qualifications

• General Requirements and credentials include a valid driver’s license and CHW certification or related training within one year preferred.
• Experience with ambulatory care management, case management or discharge planning is favorable.
• Education paths include Bachelor’s or Associate with healthcare related fields; or high school with significant experience.

Responsibilities

• Improves access to health care services through in-person interactions and referrals.
• Performs needs assessments and identifies services to meet patient objectives.
• Maintains relationships with hospitals, social service agencies and providers.
• Collects data related to referral patterns and reports on outcomes.
• Assists in resource materials and educational content for stakeholders.
• Educates internal and external stakeholders about available services.
• Interviews patients to assess needs and coordinates with PCPs on care plans.
• Maintains contact with community agencies and participates in outreach events.
