Job summary

Sparrow Health System in Lansing, MI seeks a Care Facilitator to join the Population Health Service Organization. You will liaise with Sparrow Medical Group clinics, report to PHSO, and support care coordination within ambulatory care management.

The role focuses on managing high-risk patients, chronic disease education, and evaluation of care plans with the care team to improve patient outcomes.

Qualifications

• Registered Nurse with a current Michigan license.
• Bachelor's Degree in Nursing or health care field; must complete within 2 years of hire.
• Graduate degree in nursing or related field preferred.
• National case management certification preferred.
• Experience with EMR environments (EPIC/SHS) preferred.

Responsibilities

• Provide self-management education and support to patients and families.
• Educate and support care team in self-management concepts.
• Assist patients in developing action plans and self-care goals.
• Coordinate chronic disease education for patients and families as needed.
• Advocate for patients with payers and internal resources.
• Communicate clearly with patients, families, and health care team.
• Support care coordination processes and documentation.
