Qualifications

• Bachelor's degree and 3 years of relevant healthcare experience including population health, outreach, care coordination or related programs.
• 2 years of leadership experience.
• CPHQ or HCQM certification preferred.

Responsibilities

• Manages development, implementation, and evaluation of population health strategies to improve access, equity and outcomes.
• Analyzes quality and utilization data to identify trends and priorities.
• Oversees outreach plans for Medicaid, Medicare and other populations to improve engagement and quality.
• Leads staff development, hiring, coaching and workload prioritization.
• Provides SME guidance on population health, education, and stakeholder engagement to internal and external teams.
• Develops, implements, and monitors outreach policies and procedures for program effectiveness.
• Manages budgets, contracts, and outreach activities to maximize impact.
• Supports creation and distribution of educational materials and health promotion campaigns.
• Collaborates with data teams to define metrics, evaluate performance, and drive continuous improvement.
