Job summary

Delta Dental of Michigan, Ohio, and Indiana seeks a PCDH Coordinator and Tribal Liaison in Okemos, MI. This hybrid role supports the Patient Centered Dental Home, tribal engagement, and education initiatives with internal and external partners.

The incumbent will monitor utilization, refine workflows, and ensure compliant, culturally responsive outreach to Tribal communities while coordinating with providers and MCOs to improve access to preventive care.

Qualifications

• Bachelor's degree in a healthcare-related field and 3+ years in care coordination or healthcare program administration.
• Experience working with Tribal Nations or Indigenous communities is required or highly preferred.
• Knowledge of Dental Home principles, population health, and data analysis is expected.

Responsibilities

• Administers the Patient Centered Dental Home assignment process and related program activities.
• Updates systems and workflows; handles provider and member requests and reporting.
• Acts as primary liaison to Tribal Nations and Tribal health programs; supports education and engagement.
• Promotes Dental Home strategy through outreach, coaching, and engagement activities with members/providers.
• Monitors utilization, analyzes reports, and informs continuous improvement in access to care.
• Coordinates member reassignment and tracks Tribal engagement and regulatory compliance.
